Scholarship application available here.

Scholarship applications are available to be downloaded from this website. Deadline for submission is April 1st at 5:00pm. If this day falls on a weekend or federal holiday, requests will be accepted through 5:00 pm the next business day.

Complete application packets must be received at the Foundation office by that time--postmarks will not be accepted. **Refer to the scholarship criteria listed below AND to the instructions listed on the application cover sheet. Incomplete applications will not be considered. You may copy the application to your desktop and complete it there, print an original then make copies for your packets. Do not staple, bind or place in presentation folders--simply paperclip your submissions together as individual packets.

MANY APPLICATIONS REQUIRE YOU TO COMPLETE A FAFSA (Free Application for Student Aid) AND INCLUDE THE REPORT WITH YOUR APPLICATION. YOUR ABILITY TO QUALIFY FOR STATE AND FEDERAL AID MAY DEPEND ON THE FAFSA--DON'T LOSE YOUR OPPORTUNITY FOR FUNDING BY MISSING THE DEADLINE. THE FAFSA MUST BE COMPLETED BEFORE MARCH 10th. CONTACT YOUR SCHOOL COUNSELOR FOR MORE INFORMATION.

The following is a current list of the South Madison Community Foundation Scholarships, and the corresponding elements considered in each scholarship selection process.

List of Scholarships & Criteria

Arabian Wrestling Club Scholarship

  • Must be a graduating senior wrestler at PHHS who has been active in wrestling for a minimum of 2 years
  • Must be accepted to an accredited two or four year college
  • Must have cumulative GPA of at least a 7.0 on a 12.0 grade scale
  • Demonstrate positive leadership potential in wrestling, other activities, and in the community at large
  • Must be of good character

Blake M. Bennett Memorial Scholarship

  • Must be a graduate of PHHS
  • Must be pursuing a vocational career in the area of cosmetology from any accredited cosmetology school in the State of Indiana
  • Brief explanation, not to exceed 500 words, as to why you selected cosmetology as your chosen career
  • Copy of 1 page SAR received from FAFSA showing EFC

George S. and H. Loma Berkebile Scholarship

  • Applications available at the Anderson campus of Ivy Tech State College
  • Must be a graduate of any high school in the Anderson Community School System
  • Must be pursuing vocational training at an accredited institution
  • Show evidence of work ethic

M. Brown Memorial Scholarship

  • Cumulative GPA for a graduating PHHS senior of “B” or better
  • Must be pursuing 2 or 4 year degree in the construction industry
    at an accredited institution

Brenda J.Cooksey Memorial Scholarship

  • Must be a PHHS graduating senior pursuing post-graduate vocational training or a two year associate degree at an accredited training institution
  • Eligibility is limited to graduates with a regular or Core 40 diploma
  • Applicant must have a minimum cumulative High School GPA of 6 or better on a 12
    point grading system
  • Work ethic as evidenced by summer and/or year round employment
  • Two letters of character reference; one from current/past employer and one from a
    teacher/school official
  • Copy of 1 page SAR received from FAFSA showing EFC

Kevin Carey Memorial Scholarship

  • Must be a PHHS graduating senior, active in the PHHS cross country and/or track
    running program(s)
  • Leadership
  • Academics
  • Community and School Activities
  • Sportsmanship
  • Essay (not to exceed 700 words)-“How Running Has Impacted My Life Outside
    The Sport”

Chris Coyle Memorial Scholarship

  • Active in PHHS Choral Department
  • Demonstrate moral character
  • Evidence of church, school and community commitment
  • Participation in co-curricular and extra-curricular activities
  • Showing academic potential and achievement as indicated by high school records
  • Acceptance at an accredited post-secondary institution
  • Two letters of recommendation
  • Complete required addendum

Elizabeth Edmundson Memorial Music Scholarship

  • Criteria in order of priority:
    • PHHS student who plays piano and will pursue a four-year college degree in a music discipline.
    • PHHS student who has been a member of band or choir and will pursue a four-year college degree in a music discipline.
    • PHHS student who has been a member of band or choir and will pursue a four-year college degree in education.
  • Evidence of volunteer or community service
  • Copy of 1 page SAR received from FAFSA showing EFC
  • Essay (not to exceed 500 words) – “How Music Has Impacted My Life”

Jennifer C. Fox Memorial Scholarship

  • Cumulative GPA for a PHHS graduating senior of between 5.0 and 9.0
    on a 12 point scale
  • Must emulate Jennifer’s freedom of thought and love of life
  • Acceptance to an accredited Indiana public college/university
  • Ability to clearly express thoughts and ideas through effective writing
  • Academic performance and achievement in liberal arts
  • Two letters of recommendation; one personal and one academic
  • Complete required addendum

Charles L. Godsey Memorial Athletic Scholarship

  • Must be a graduating senior active in the Pendleton Heights High School sports program
  • Cumulative GPA of 7 or better on a 12-pt. scale
  • Must not be recipient of athletic scholarship(s)
  • Evidence of mental attitude and work ethic
  • Two reference letters; one character and one coach’s

Donald Gwinn Memorial Scholarship

  • Must be a graduating senior of Lapel High School and accepted to an accredited
    four year institution of higher learning
  • Cumulative high school grade point average of “B” or better
  • Evidence of a strong work ethic, active community service and career plans in an
    Ag-related or scientific field
  • Essay (500 words or less) outlining future goals in agriculture or science

Tyler J. Hatheway Memorial Scholarship

  • Must be a graduating senior of PHHS and accepted to an accredited four year institution of higher learning
  • Cumulative high school grade point average of “B” or better
  • Demonstrate consistent, long-term & regular involvement & dedication to their local church & show evidence of strong & favorable high character & evidence of care & concern for others (a “team player”) – qualities that are to be addressed by a reference letter from their Pastor
  • Essay (250 – 500 words) – EITHER "Excluding your immediate family, who in your life has had the most influence on your character and why?"  OR “When your life draws to a close, what criteria will you use to measure the successfulness of your life and why?"
  • 2 additional letters of character recommendation from someone other than family members
    -Complete required addendum

Jessica D. Henson Memorial Scholarship

  • Must have been active in the Pendleton Jr. Baseball/Softball Association Program
  • Evidence of church, school and community commitment
  • Show academic potential and achievement as per high school records and plans to pursue a degree at an accredited institution
  • Two letters of recommendation; one personal and one academic
  • Copy of 1 page SAR received from FAFSA showing EFC
  • Complete required addendum

Jack and Helen Hobbs Scholarship

  • Fall Creek Township resident
  • Show profit by a vocational or college education at an accredited institution
  • Copy of 1 page SAR received from FAFSA showing EFC
  • Complete required addendum

J.K.L. Science Scholarship

  • Graduating from PHHS
  • Cumulative GPA of between 8.0 and 10.0 on a 12 point scale
  • Active interest in the study of physical science/life science
  • Must be accepted to an accredited 4 year college/university pursuing studies related to the physical/life sciences
  • Must actively participate in co-curricular school and community activities

H. Ross Jackson and Elizabeth S. Jackson

  • Must be a graduating senior at PHHS pursuing a four (4) year degree in education
    at an accredited institution
  • Applicants must have a G.P.A. of “B” or better

(Karen R. Brown and) Joseph B. Keene Scholarship

  • Applications available at the Anderson campus of Ivy Tech State College
  • Must be a Madison County Resident pursuing a degree in Practical Nursing who intends to practice in Madison County

Patricia H. Kline Memorial Scholarship

  • Must be in the top 25% of the PHHS graduating class
  • Must be entering the field of social work, religious studies or performing arts
    at an accredited institution
  • Active in extracurricular (dance/theatre/choral/band) and church-related activities
  • One page essay entitled “Two Lessons I've Learned from My Parents That I Will Pass To My Children and Others"

Paul C. Kline Memorial Scholarship

  • Must be in the top 25% of the PHHS graduating class
  • Must be entering the field of engineering or engineering technology at
    an accredited institution
  • Active in extracurricular and church-related activities
  • One page essay entitled "How My College Studies and Degree Will Make A Difference in My Community"

Kelson L. Lineberry Memorial WEEM Scholarship

  • Cumulative GPA of “B” or better
  • Active in WEEM radio program at PHHS

Jill D. Mollenkopf Memorial Scholarship

  • Must be a PHHS female graduating senior
  • Must have played four continuous years of high school softball
  • Must have a varsity letter at least 2 of the 4 years
  • Acceptance to an accredited post-secondary institution
  • Copy of 1 page SAR received from FAFSA showing EFC

Sandra A.Neese Memorial Scholarship

  • Must be a graduate of Lapel High School
  • Priority will be given to students who demonstrate an interest in public service, law or volunteering for organizations such as the Peace Corps or Americorps
  • Cumulative GPA of 2.5 or higher on a 4-pt scale
  • Evidence of leadership through community service and/or extracurricular activities
  • Two letters of reference: one addressing academic success and potential and one
    addressing the applicant’s character
  • Essay, not to exceed 500 words, explaining why you think you deserve the scholarship

PHHS Memorial Scholarship

  • Graduating senior of PHHS
  • Demonstrate potential to complete undergraduate studies at an accredited institution
  • Extracurricular activities and community service

Pendleton Optimist Club Scholarship

  • Cumulative GPA of “C” or better for a graduating PHHS senior
  • Must be pursuing additional education through vocational/technical training

Randolph/Fraternal Order of Police Scholarship

  • Must show proof of being a senior of a parent/grandparent who are members of F.O.P. #48 as provided by the FOP Lodge #48
  • Must be a senior from Anderson, Highland, Lapel, Pendleton, Alexandria, Elwood, or Frankton High Schools
  • Must be accepted at an accredited college/university pursuing an undergraduate degree
  • Copy of 1 page SAR received from FAFSA showing EFC

Jeremy Rodecap Memorial Scholarship

  • Must be a graduating senior of the South Madison Community School Corp. and/or a
    resident of Fall Creek, Adams or Green Township, Madison County, IN pursuing post
    high school training in the field of Emergency Medical Services, Emergency Medical
    Technician or Paramedic. (or second choice of education in the Medical Service field)
  • Training must take place at an accredited institution
  • Cumulative GPA of 2.5 or higher on a 4-pt scale (graduating seniors only)
  • Evidence of community service and active church affiliation
  • Typed essay of 500 words or less outlining career goals
  • Two letters of reference; 1 personal and 1 character
  • Copy of 1 page SAR received from FAFSA showing EFC

Andrew D. Shoup Memorial Scholarship

  • Must be a member of the PHHS baseball team
  • Demonstrate exemplary character and citizenship characteristics
  • Evidence of a strong work ethic
  • Demonstrate a passion for the game of baseball
  • Academic aspiration and achievement as indicated by high school records with plans to pursue vocational training or college degree at an accredited institution
  • Two letters of recommendation
  • Complete required addendum

Howard L. and Elva Fern Simmerman Memorial Scholarship

  • Pursuit of a 4-year education degree at an accredited college/university
  • Cumulative G.P.A. of 2.5 or higher on a 4.0 scale
  • Evidence of Leadership through Community Service and/or Extracurricular activities
  • Work Ethic
  • Copy of 1 page SAR received from FAFSA showing EFC
  • Priority to PHHS graduates enrolled at an accredited college/university pursuing an education degree

Fred St. John Memorial Scholarship

  • Must be a graduating senior at Lapel High School
  • Must be accepted to an accredited two or four year college or vocational institution
  • Evidence of Leadership through Community Service and/or Extracurricular activities
  • Copy of 1 page SAR received from FAFSA showing EFC
  • Submit a brief explanation (250 words or less) of future and career goals

Ruth Willard Scholarship

  • Graduating senior of PHHS
  • Must have work ethic
  • Ability to profit by college education
  • Pursuit of a 4-year degree at an accredited college/university
  • Copy of 1 page SAR received from FAFSA showing EFC
